Do musicians' earplugs actually work?

Yes — a randomised trial demonstrated it. The problem has never been whether they work, only whether they get worn.

The trial

A randomised controlled trial assessed earplug effectiveness against recreational noise exposure [1]. The result is unsurprising and worth having: attenuation at the ear reduces measured hearing change. What the trial cannot fix is the compliance problem.

Earplug effectiveness

Why foam plugs fail

Foam plugs attenuate high frequencies far more than low ones — often 15 dB more at 4 kHz than at 125 Hz. The result is a muffled, bass-heavy sound that makes mixing genuinely harder. You cannot hear the hi-hats, so you take them out.

Filtered musicians' plugs use an acoustic filter to attenuate roughly evenly across the spectrum. The music sounds like the same music, quieter. That is the entire design goal, and it is the difference between protection that is worn and protection that lives in a pocket.

Choosing the attenuation

More is not better. Over-attenuating in a booth means you cannot hear the room, so you raise the monitor, and you have moved the exposure rather than reduced it. For most DJ work, filters in the 15-20 dB range preserve enough room information while bringing dose within a defensible range. Custom plugs with interchangeable filters let you match the venue instead of guessing once.

In-ear monitors

A well-fitted in-ear monitor provides isolation and lets you set your own level, which is theoretically ideal. In practice they introduce a different failure mode: with the room excluded, people frequently run their own mix loud, and the exposure returns. Common questions

How much attenuation do I need?
For most club work, 15-20 dB of flat attenuation brings exposure within a defensible range while leaving the room audible.
Are custom plugs worth the money?
They fit better, sound better and get worn more. Since the failure mode is not wearing them, that is the decisive advantage.
Can I use in-ears instead?
Yes, if you control your own level. Isolation without level discipline just relocates the exposure.
Do they make mixing harder?
Flat filters, barely, after a short adjustment. Foam plugs, considerably — which is why people abandon them.
